Keio Infectious Diseases Fellowship Program

1. Program Overview

This program centers on the pillars of “Clinical Practice × Infection Control × Research × International Perspective.” It aims not only to prepare participants for certification as specialists by the Japanese Society of Infectious Diseases but also to cultivate infectious disease physicians capable of global engagement. Beyond advanced clinical skills and hospital infection control capabilities, the program develops the ability to independently plan and conduct clinical research, translational research, and physician-initiated clinical trials, aiming to produce future leaders in the field.

Furthermore, this program offers career paths leading to a Master of Public Health (MPH) or Doctorate (PhD), supporting the development of professionals with high expertise in both clinical practice and research. Flexible career support enables the realization of diverse career paths within the field of infectious diseases.

Physicians who have obtained or are scheduled to obtain board certification in a primary specialty

Physicians who have obtained board certification in an internal medicine subspecialty and aim to obtain board certification in infectious diseases

A sequential certification program for internal medicine and infectious diseases specialists is currently under development.

①Cross-Organ Medical Care

We respond to consultations from all departments and provide medical care for infections affecting all organs. Even in cases where a patient does not ultimately have an infectious disease, we aim to contribute to the quality of care. The goal is not solely to treat infections, but to strive for the best possible outcome for each individual patient.

臨床微生物学

②Clinical Microbiology

Since infectious diseases are caused by microorganisms, understanding them is essential. You will acquire clinically important microbiological knowledge, such as identifying the causative microorganism, methods of identification and their uncertainties, and what to do when identification is not possible. This competency also includes interpreting Gram-stained smears of clinical specimens. The training is conducted in collaboration with the Department of Clinical Laboratory Medicine at Keio University Hospital.

③Pharmacokinetics

There are various drugs used to treat infectious diseases, and their appropriate use requires proficiency. Proper use of antimicrobial agents is crucial to suppress the development of drug-resistant bacteria. You will gain knowledge and skills akin to an "antibiotic sommelier," including how to select the right antimicrobial agent, different methods of administration, side effects, drug interactions, and alternative treatments when first-line drugs cannot be used.

④Ethics in Infectious Diseases

Infectious diseases raise numerous ethical issues, such as the necessity of isolation to prevent disease spread or the disclosure of HIV or drug-resistant bacteria carriage. Isolation can limit individual rights, and Japan has a history of prejudice and discrimination related to infectious diseases such as Hansen's disease and HIV. You will learn to develop an ethical perspective that balances public health with the human rights of individual patients.

⑤Emerging and Re-Emerging Infectious Disease Countermeasures

Globally, various emerging and re-emerging infectious diseases, such as novel influenza, Zika virus, COVID-19, and monkeypox, continue to arise. You will acquire the ability to understand global epidemiological trends and develop necessary countermeasures for Japan and hospitals. Additionally, you will cultivate the skills to stay current with the latest medical information, thoroughly review relevant literature, and independently consider the best treatment and infection control measures.

⑥Infectious Disease Crisis Management

Infectious disease control involves both routine measures, such as hospital facility management, surveillance, hand hygiene, and adherence to PPE usage, and outbreak-specific responses, such as patient isolation, active epidemiological investigation, and ward closures. Crisis management also includes handling media communication during hospital outbreaks. While most hospitals have infection control departments, you will learn how infectious disease specialists can play a vital role in infection control.

B. Clinical Research and Translational Research

・Clinical epidemiology research (observational studies, prospective cohort studies, registry studies)

・Planning physician-initiated clinical trials, IRB applications, and monitoring practices

・Participation in multi-center collaborative research and international collaborative research

・Genome analysis and multi-omics analysis (in collaboration with laboratories both within and outside the university)

C. Diverse Career Support and International Expansion

  1. MPH Course (in collaboration with Keio University Graduate School of Health Management: includes evening and Saturday-focused working professional courses)

Environment Fostering High Clinical Competence:

Keio University Hospital receives a high volume of consultation requests for difficult-to-treat cases and severe infections, providing practical opportunities to hone diagnostic and decision-making skills as an infectious disease specialist. This environment offers extensive exposure to highly specialized cases, including bloodstream infections (particularly managing blood culture-positive cases), infections in immunocompromised patients, and infection control for multidrug-resistant organisms. By collaborating with multiple disciplines as an infectious disease consultant and actively participating in infection control and antimicrobial stewardship support at the point of care, trainees develop skills that make them immediately effective in clinical settings.

Strong Research Capabilities and Support System:

We have collaborated with numerous PhD researchers in cutting-edge infectious disease research fields, such as mycobacteria, COVID-19, and vaccines, to advance practical clinical and translational research. For example, we perform flow cytometry analysis using patient specimens and pathogen genome sequencing, offering integrated laboratory-based training that embodies the “bedside to bench, bench to bedside” approach. We also conduct dry lab research, including large-scale database studies using claims data and health economic evaluations (cost-effectiveness analyses). Furthermore, under a robust support system collaborating with the Research Promotion Center and the Biostatistics Team, we provide comprehensive assistance from paper writing and research grant applications to international conference presentations. We cultivate infectious disease physicians with the ability to “independently forge their own path”.

Strong Infection Control System and Team-Based Care:

Led by the Infection Control Department, which includes a dedicated pharmacist, three Infection Control Specialist Nurses, and one Certified Infection Control Nurse, we implement infection control measures at a high standard within Japan. A major appeal of this program is the opportunity to learn within practical, multidisciplinary team-based care.

Flexible Program Design:

We are developing combined training programs in infectious diseases through collaborations with various domestic medical institutions and universities. This allows for flexible research program design tailored to individual career paths.

In recent years, our department has pioneered new initiatives, utilizing public funding to expand infectious disease research beyond Japan into overseas field settings.
